Window regulator problem

I have an issue with my drivers side window regulator that I am hoping to get some help with.
 
When the window is wound all the way to the top, firmly enough to push the window into the channel, the slide on the regulator drops out the front of the rearmost channel of the window. When I put the slide back in it is a fairly simple matter to wind the window up and align the two and slot it back in the front. I think it is too easy to fix/come apart.
 
Is this an adjustment issue? It appears to be adjusted correctly but I claim no expertise aside from all the holes are good.

    It is adjustment.  I had a B%%%%R of a job realigning the glass in the door on rebuild.
    You need lots of patience and some time to slowly tweak the adjustment. Trial and error I'm afaraid.

    The front mount (winder end) has four bolts. can't get that wrong.
    The rear mount(3 bolts) on the inner door face has a slot for one bolt. This is the one to get adjusted.
    I don't recall if the rear mount only fits one way (offset bolts). Being the rear pivot point, correct bolt placement is important to ensure the rotation of the arms happens around the correct point of rotation relative to the door frame.  (hope that gobbledegook makes sense)
    The arms also need to be in the correct starting position or it just wont work !!!! Can't describe any better than that.

    Cheers heaps for the replies. I will have to have a tinker. It pretty much seemed like once the bolts were in on the rear that was it.

    i tried fitting mine tonight and as mentioned can be a **** of a job.. i found not only do you need tweaking the window mech but you also need to tweak the window frame as well
