Another area to look at with shifters is the thread in the extension housing. When the nylon insert loosens,it wears the alloy in the centre of the threaded portion. No matter what type of shifter you use,or how well it is fitted,will only stay put for a short time.
I found this with the single rail BW in my 351 XD falcon.
I rebuillt the shifter 3 or 4 times with replacement threaded insert,fitted 2 brand new Ford shifters with no success. It was only after I replaced the housing at a later date that I was able to fix it properly.
